427 Exhaust Cooling Sys. or Head Gasket

Does the exhaust cooling manifold have-to-have an internal crack to leak water into a cylinder? Standing water in # 4 and moisture in #3,2,1. I found one bolt loose and several that I could get some more turn on (1/16 turn). I'm going to re torque (spec's 12-18 lbs.) and see if this solves my problem. This is a new BARRS manifold and gasket just installed four months ago and has 10 hrs. run time. Replaced the head gasket at the same time. I do not know if the mechanic used proper torque, or sequence.
The cooling manifold appears to be a "closed loop system" with the only way water could get into the exhaust chamber would mean manifold internal crack ... right? Why moisture in all four cylinders on the stbd. side? Number 4 is flooded and plugs wet in 3,2,1.
